the challenge
Traditionally, data collected by hundreds of caseworkers on homeless individuals’ health, living situations, and needs had been captured in siloed databases and case files, making it difficult for workers to share information with each other and community-based organizations.

the solution
DHS selected Nagarro to design and implement a modern, mobile case management system with a unified platform to manage homelessness across New York City. The new application enables homeless outreach workers across the five boroughs to collect data in a single citywide database in real-time and avoid duplicate record. It also features a map that helps outreach teams track the locations of encampments and assess the effectiveness of various housing facilities.

the outcome
By integrating data from different city and state systems, duplicate records are avoided, ensuring the accuracy of individual case records even when individuals change boroughs. Consistent and efficient outreach is essential for getting people off the streets, and this application helps workers achieve precisely that goal.
